If you are interested in adopting a cat or kitten from or shelter please fill out and submit the application below.  

Please be aware that filling out the pre-adoption application does not guarantee adoption of the cat. This form gives us your information and lets us know you are interested in adopting from SHS. We recommend that you follow up your application with a call to set up an appointment to meet the animal you are interested in. 

When we receive multiple applications for the same cat, our policy is to choose the home we feel is best suited for the cat’s unique needs.  We can not hold available cats for more than 24 hours for any potential adopters.

In accordance with Animal Welfare Law, all cats and dogs must be vaccinated for rabies.
Please understand that SHS cannot guarantee the behavior or health of this animal. If you are approved for adoption, are you prepared to make this kind of commitment of your time and money? (yes or no)
If you adopt this cat please understand that the adoption fee is non-refundable. If you decide that you need to return this cat to SHS, you have 10 days to do so without being required to pay the standard surrender fee of $25. We do require a phone call before you return the cat to assure that we have available space to take it back. (yes or no)
Our goal at SHS is to place animals into permanent, loving homes; therefore filling out this application does not guarantee adoption. Please understand that if you are refused for this adoption, it is because it is in the best interest of the animal. Upon adoption, you must provide a carrier for your new cat, or we can offer you a cardboard carrier for only $5. All cats must leave the building in a secure carrier. By typing your name and checking below you have stated that all the information on this application is true to the best of your knowledge.
