This form will give us the information to put into our shelter database. If you have recent photos of your pet, we can add them to the report. Email them to frontdesksomerseths@gmail.com, with your name and the pet’s name for reference. When you find your pet, please email or call us to let us know so we can change your pet’s status to “found”. We keep lost reports on file for 6 months. If your pet goes missing more than once you will need to fill out a separate report each time so we have the most accurate information.
